Across the US, chronic knee pain is a growing concern. In recent years, the rate of knee replacement surgeries has increased 8 times among those 65 years and older21.

In many cases, especially among older adults, the reason for chronic knee pain is osteoarthritis (OA) of the knee. Osteoarthritis is the single most common cause of disability in older adults22, and according to the National Institutes of Health, knee osteoarthritis has doubled in the US since the mid-20th century23.

However there are a number of other conditions—related to the meniscus, patella, and ligaments—that also contribute to the widespread occurrence of chronic knee pain.

There is often more than one reason for chronic knee pain, as it is a wide umbrella term. Some of the more commonly used definitions for chronic knee pain include knee osteoarthritis, torn meniscus, chondromalacia patella, patellofemoral syndrome, patellofemoral arthritis, iliotibial band syndrome and knee ligament damage (such as acl, lcl and mcl tears and strains). 

Knee conditions can be acute (lasting less than 3 months) or chronic (lasting or recurring for more than 3-6 months). They can also affect different parts of the knee joints and be felt throughout the leg. Problems with joints above and below the knee may also refer to the knee.

Knee pain conditions can often be diagnosed by your physician or physical therapist by carrying out functional tests and discussing your symptoms.

Symptoms associated with chronic knee pain include tenderness, swelling, stiff knee, loss of movement and, of course, pain. These symptoms can be experienced throughout different parts of the knee – on the inside (medial knee pain), outside (lateral knee pain) or on both sides (bilateral knee pain) – and the pain can be present both at rest and during activity. Chronic knee pain can affect people of every age group.

The knee joints are complex, so multiple factors can harm them and the surrounding soft tissues. These include wear-and-tear from aging, overuse, excessive flexibility, bad knee alignment, weak muscles and injury.

Risk factors that can increase a person’s chances for developing knee pain include obesity, genetics, gender (women are more likely to develop OA), overuse of the joint due to work or sports and previous joint trauma.
